Celiac Disease and Gluten Free Diet

Celiac disease covers a spectrum of illness from ambiguous, relatively mild digestive symptoms to more destructive and even life-threatening disease. The digestive tract is injured by gluten proteins and does not work well. Celiac disease occurs at all ages in both males and females. The prevalence of celiac disease in the general population was believed to be 1 in 300, but recent evidence suggests that there are more undiagnosed than diagnosed cases. The prevalence of CD among first-degree relatives is as high as 22%.

If you ask how many people in the general population suffer when they eat gluten? there is no accurate answer. If you ask how many people will improve if they change their diet excluding gluten? ...again there is no real answer to the question.

We believe that all forms of gluten allergy may cause illness in more than 20% of the general population. Most gluten-sensitive patients have not been diagnosed. We argue that any person with celiac-like symptoms should do diet revision right away and not worry about an exact diagnosis.

In celiac disease the gastrointestinal tract is the primary target organ; however systemic disease is an important consequence of cereal grain ingestion in many patients. We think that the people diagnosed with celiac disease are a sub-population of a much larger group with gluten allergy. 

Often, an assortment of related whole-body problems accompanies celiac disease. Gluten and other food antigens make their way through human bodies in a remarkable fashion. Consider the long and improbable path of milk proteins through a mother's gut, into her blood, through her liver, out into her breast milk, through her infant's gut mucosa and into the infant's nasal mucosa to cause rhinitis, the infant’s lung to cause asthma, or the infant’s skin to cause eczema.

We think the related problems are typical of delayed patterns of food allergy and use celiac disease  to create a model of delayed pattern food allergy. Celiac patients have increased gastrointestinal permeability and demonstrate the whole-body effects of food allergy, including brain dysfunction, arthritis, and inflammatory lung disease. 

Despite intensive study, the mechanisms of gluten hypersensitivity diseases have not been finally determined. The pathogenic mechanisms appear to be multiple and complex. The term "allergy" means the same as hypersensitivity and both terms refer to immune mediated disease. Celiac disease involves delayed hypersensitivity. Some patients also have immediate hypersensitivity reactions to gluten proteins.

A cell-mediated type of  gluten allergy leads to a host of complicated consequences, especially increased permeability of the gastrointestinal tract. The surface lining of the digestive tract is the largest and most critical interface between you and your environment.

In the small intestine, the lining epithelial cells are involved in immune processes. They transfer antibodies  produced by B-lymphocytes to the surface and interact with other cells of the immune system to induce an inflammatory response to stop microbial invasion. The surface epithelium processes food antigens, and presents antigens to lymphocytes.

When the diagnosis is first made, a gluten-free diet is prescribed. Wheat, barley, rye, and oats were excluded for many years on  gluten-free diets. Rice is the principle substitute for cereals. As a matter of policy, the Alpha Nutrition Program continues to leave out wheat, barley, and rye. Oats is an optional food that is introduced late in the program to assess acceptability for each individual.

Gluten Free: The exclusion of wheat, rye, barley are the initial steps when gluten allergy is suspected.  Gluten elimination should be part of a more comprehensive diet revision plan, preferably in the form outlined in the Alpha Nutrition Program.
